Registration

To register:

Every youth should receive this form from their ward. You may also print and fill out this form on your own. This form is required to participate in Trek. By completing the following registration form below you give permission for your child to participate in the 2024 Vineyard Stake Trek event and authorize the adult leaders supervising this activity to administer emergency treatment to the participant below for any accident or illness and to act in your stead in approving necessary medical care if you cannot be reached.  This authorization will also cover travel to and from this activity.

Please turn in the paper form to a member of your bishopric or young women's presidency by April 1.
